The math of professional well-being for docs and nurses is astounding... 900,000?

Replacing healthcare workers is costly to an institution. Replacing a physician is quite costly, one study suggested it cost 900,000 to replace one doc! Another study suggested it costs 40,000 to replace each nurse. The cost varies by specialty and area of the country. Nurses and physician burnout rates are quite alarming and seem to feed off each other. A recent study showed when nurses are not doing well, docs and nurse turnover rates are higher. It makes sense that we are all connected at work, in the joy and the pain. The study mentions that both docs and nurses rate the management’s solutions to care delivery as their priority. Both docs and nurses stated < 10 % have a joyful environment at work (Aiken, JAMA Forum 2023 Jul 7;4(7): e231809.) Becker states hospitals have turned over 86% of their staff since 2016, which is also quite surprising to me. The 2020 rate for RN turnover was 18.7% at approximately the cost of $40,038.00 per nurse. The hospitals could possibly lose 3.6 million in a year as the percent change in RN turnover could cost or save a hospital 270,800 per year. Do we wait and spend the money on replacing the docs and nurses or do something to support them now? The well-being of our healthcare workers is vital and has a cost. Shall we be preventative or reactive?

The American Medical Association, AMA, in 2017, estimated it costs 800,000 or more to replace a doc due to lost wages. Turnover of physicians is also disruptive to patients, families, staff and organizational culture. The American Medical Association has been prioritizing burnout prevention since before COVID. Multiple times a week I hear from my patients that their docs have left and they cannot get into a medical appointment for months and months. How do we support the healthcare workers that are supporting our communities?

“To help physicians succeed in their life's work of caring for patients”, the AMA has made the prevention of burnout a core priority. Working with partners across health care, the AMA is leading a necessary change in medicine that embraces physician well-being as a critical factor in the long-term clinical and economic success of our nation’s health care system.” The American Medical Association made this statement prior to COVID. We are all still wondering what is the optimal solution for docs and nurses?

What does it cost to support a physician and keep them in the workforce?

Wellness practices vary as do their costs.

The solution to burnout is not apparent at this time as the numbers of docs and nurses leaving or considering leaving is rising in 2023.

We went into medicine to serve, learn, innovate, challenge ourselves and support our communities. Yet so many are walking away as they do not see an option for well-being and staying in medicine. How do we help folks see more options?

We, in medicine, need to look at other industries that may have solutions that we can use or fit to our needs. The military was involved in a huge multimillion dollar study, in 2011 during the heat of the active war in Afghanistan, looking at ways to prevent burnout and post traumatic stress. They tested resiliency education, online courses and training the trainer with a large team working on well-being and positive psychology lead by Dr. Martin Seligman. Dr. Martin Seligman is referred to as the “father of positive psychology”, he pioneered the focus of using psychology to keep folks well rather than only studying the things that went “wrong” with the mind. Interesting results were seen, less post traumatic depression, anxiety and substance abuse was decreased by 50% post deployment when the soldiers had resiliency training. The General at the time said let’s put everyone through this as soldiers were serving multiple tours in Iran and Afghanistan, he was ready for preventative care of his soldiers. What lessons can we take from this? The military had the foresight to put money and attention toward a problem preventatively and saw positive results. It was not that they did have a war to fight, they still had very tough work. They prepared their teams and leaders with new training that showed it helped their teams on the other side of the horribly stressful work. It seems to me that there are parallels with medicine here. We are waiting for the admin to solve the challenges in medicine yet we are still in the “trenches” day to day. What if we were preventative, similar to the military, and trained our leaders and teams on simple positive techniques that could help us with tough work?

In medicine, we do not have one program we put everyone through at the start of medicine or after so many years of practice. We have innovators and large national associations working on decreasing burnout yet it has been reported since 1974. Could we look at other industries that seem to be helping their workers rebalance and learn from other’s tests and successes?

What would it cost to proactively educate folks entering medicine with a proven program? Since it costs 800,000 at least to replace one doc, it seems like the math would make sense to figure this out.

I have been involved, with an amazing team of OB/Gyns, in an IRB approved study looking at resiliency and self efficacy rate of OB/Gyn residents after access to an app based Resiliency Well-Being Program that I designed with Ali Hively. We united our talents and passions and brought new education and tools and mixed it with compassion and grace. We did not have a control group yet we did see a correlation with a boost in resiliency and self efficacy after compassionate science based education. Those involved were working hard and supporting Moms and new lives in Ob/Gyn. The program gave them a few new tools to fortify them to keep them able to use all their skills to thrive in medicine. We need docs that are well prepared to thrive in medicine. We also need research studies more to know the best steps to support our healthcare workforce.
